dc.description.abstractMore than 20 years after the publication of the Butler report which led to the establishment of medium-secure units, many of its recommendations have not been implemented. There is still a shortage of beds in medium-secure units. A survey of 18 of the 25 units in England and Wales has revealed wide variations in size as well as staffing ratios. National leadership and more coherent planning is needed.
