We present the case of a 35-year-old patient suffering from an untreated solitary osteochondroma and subsequent rare transformation to a dedifferentiated chondrosarcoma. This case highlights the potential risk of malignant transformation and raises the question of patient and clinician responsibility in identifying changes in a premalignant condition.en
